Connection Issue - Possible Port Problem

I have lost connection to my HP EX490 via RDP and WHS console. Also no network shares visible from other computers. Pings on server name and fixed IP work fine. After reading some similar issues here I have run the Toolkit and have attached the log.

The only thing that sticks out to me is the connection to port 8912. The router has it listed under port redirection to the correct fixed IP but it is listed twice. Once under UDP and once under TCP?

Is there anything else there that could cause the issue?
